They will fax them for you -- free of charge.

10:25:35 AM [Amy Story]  Child Care Centers are licensed by the Ohio Department of Job and Family Services. Call 1-866-635-3748 Option 4 to report concerns regarding licensed child care facilities.

10:21:35 AM [Amy Story] If you are interested in becoming a Limited or Professional Type B Home Provider, you need to obtain an application to complete. Applications are available at 222 E. Central Parkway. We're located on the Fourth Floor, but will be moving to the Third Floor on Oct. 6.

10:08:41 AM [Visitor] My husband and I are caring for two of our grandchildren (ages 5 1/2yrs and 19mos) and have POA, but our daughter (their mother) still has legal custody. When her eligibility for childcare vouchers is up for renewal, will everything still be based on our daughter's information? She's usually MIA and not consistent with caring for them..which is why we have POA. My husband and I work full time, but still cannot afford the cost of full time daycare for the two kids without the assistance.
10:09:35 AM [Amy Story] Child Care eligibility is not based on legal custody. If the children reside with you and you are their current caretakers, you should apply. Your eligibility will be based on your family size and income..

10:03:35 AM [Visitor] My daughter applied for child care benefits for her daughter while she attends UC. A statement of support was provided from me, however she has yet to hear if she has been approved. What is the hold-up and how can this be resolved quickly? UC started classes today!

10:04:35 AM [Amy Story] When did your daughter apply? We're currently experiencing a three-week delay for new intake cases.

8:06:11 AM [Visitor] Ok, does a child support felony on someones record prevent that person from becoming a substitute/emergency caregiver or even living at the residence where child care is provided?
8:53:21 AM [Visitor] Sidenote: Also the child support case is closed/terminated but the person hasn't had it(the felony record) expunged off their records as of yet.
9:12:17 AM [Amy Story] Both adult household members and emergency substitute caregivers must have a criminal records checks completed.  HCJFS could not approve  an individual as a substitute caregiver or a provider should they have a disqualifying offense on their record.  The provider would also not be approved if a household resident had a disqualifying offence.  A child support felony however is not a disqualifying offense.
