Permission to Administer Medication (JFS 01644)

1.    Print a copy of the form for each child
       and each medication.

2.    Have the caretaker, parent, or physician
       complete as needed.

3.    Keep the original forms in your files. Your
       Home Provider Specialist will ask to see
       these forms during home visits.
