What is Medicaid?

Medicaid is state and federally funded health care coverage. HCJFS administers Medicaid programs that provide health care coverage to:

  • Income-eligible parents and their children up to age 21

  • Preventive and treatment services up to age 21

  • Pregnant women

  • Older adults

  • Income-eligible people with disabilities

  • Children in foster care

Medicaid is very broad, with two major covered groups, Aged, Blind, and Disabled (ABD), and Covered Families and Children (CF&C).

There are several Medicaid programs:

  • Healthy Start is geared toward pregnant women and children up to age 19. Eligibility is based on income and family size.

  • Healthy Families helps children under age 19 and parents with incomes at or under 100 percent of the federal poverty level.

  • Healthchek provides preventive and treatment services for anyone eligible for Medicaid under age 21.

  • Pregnancy Related Services refers Medicaid-eligible pregnant women to prenatal care and transportation services.

  • Aged, Blind and Disabled Medicaid (includes nursing home and in-home care).

Healthy Start/Healthy Families:

To apply for Healthy Start or Health Families, print and complete this application. It can be either mailed or faxed.

NET (Non-Emergency Transportation)

HCJFS customers who are receiving Medicaid assistance may be eligible for transportation services. To be eligible, customers must:

  • be Medicaid customers in Hamilton County

  • request transportation ONLY to a Medicaid provider

  • not be a resident of a long-term care facility

  • not be pregnant

  • be able to transfer from a wheelchair to vehicle independently
