Verification forms added to Web
July 16, 2008 -- Medicaid, Ohio Direction Card and Ohio Works First (OWF) clients no longer need to travel downtown to pick up or drop off two widely-used forms for household and employment verification.

The Hamilton County Department of Job and Family Services this week added the forms to www.hcjfs.org -- under Quick Links/Applications-Forms and on sections for the various programs under the Services button.

After signing the forms and taking steps such as having a landlord or employer complete them, customers can then take them to a branch of the Public Library of Cincinnati and Hamilton County for free faxing. Or they may mail or drop off the form at the agency.

Here are basic steps after opening the form on the Web site:

(1) Print the form
(2) Sign your name in the appropriate section
(3) If you don't know your caseworker's name, district number or fax number, please call (513) 946-1000 and an Information and Referral Specialist will assist you.
(4) Take the form to your landlord or employer to be completed.
(5) Before returning the form, make sure you write your Social Security Number or Case Number on the top of the form.   
(6) You may return the form by mailing it or dropping it off at one of our Document Drop Off Centers located at 222 East Central Parkway or 237 William H. Taft Road. You can also fax it to us free of charge at any branch of the Public Library of Cincinnati and Hamilton County.  

As part of ongoing customer service improvements, the agency also recently added an online Report a Change form to the Web site as well as online Contact Us forms for various program areas.

Other forms and applications have been on the site for several years.

Plans call for adding more.

Senior Services and Caregiver Night at Great American Ball Park
Discounted tickets, pre-game festivities, senior services information
July 8, 2008 -- Social workers, caregivers and others who assist senior citizens will be honored at the Cincinnati Reds-Houston Astros game Friday, Aug. 8, at Great American Ball Park.

Discounted tickets are available for Senior Services and Caregiver Night. Click here for an order form.

A group of seniors and their caregivers will be honored during pre-game festivities. They also will call "play ball" to start game action.

Information about services for seniors will be available at the ball park. There will be a fireworks display after the game.

There also will be informational books at the July 23, Aug. 6, and Sept. 2, 4 and 18 games.

Foster care and adoption Open Houses planned
July 7, 2008 -- Starting Aug. 11, people interested in foster care and adoption can get information at a monthly Open House, 6-7:30 p.m., at 237 William H. Taft Road.

Open Houses offer people interested in foster care and adoption the opportunity to get information outside of a classroom setting, take it home and decide if fostering and/or adopting is right for them.

Other events are scheduled for Sept. 15, Oct. 13, Nov. 10, and Dec. 15 -- same time and location. 

State solicits comments about child support guidelines
July 7, 2008 -- Citizens can comment online or at a public meeting on guidelines that help determine the size of child support payments. State law requires a review of the guidelines every four years to ensure they adequately provide for the needs of children involved in child support cases.

  • To comment in person in Southwest Ohio, attend a hearing, 11 a.m.-1 p.m. or 5-7 p.m. Monday, Aug. 18, at 237 William H. Taft, Room GN009. Hearings also are scheduled at other locations throughout the state.
